About Payment:
Payment is due at the time services are delivered unless prior arrangements are made. Payment may be made by cash, personal check or credit card. There is also a payment option (via credit / debit card or PayPal) on this website (click here)
About Health Insurance:
The services I provide are covered by many health insurance companies. Although I do not process insurance claims, I can give you the necessary paperwork so that you may file a claim to request for reimbursement from your insurance company. It is your responsibility to determine if your insurance will reimburse you for my services.
Note to Medicare Beneficiaries:
I am an Opted-Out provider. This means Medicare will not reimburse you or me for my services. I am permitted to treat Medicare Beneficiaries only if you and I enter into a private contract which states that you understand you will not be reimbursed.
This means that you will be responsible for the entire cost of my services.
About Cancellations:
Sessions canceled or missed within 24 hours of the scheduled meeting time will be billed to you at the full fee regardless of the circumstances. Insurance companies will not pay for missed or canceled sessions. If another appointment is available the same day and you can change to that time you will not be charged for a missed appointment.
